Medication Forms

In order for medication to be administered in school, the relevant medical form must be completed. No medication can be issued in school without the appropriate form. Please see below a flow chart to help you determine which one to use and a link to the forms. Please contact the school if you are still unsure which medical form to use. Copies of the forms are also available at the school office.
