Medication Forms

In order for medication to be administered in school, the relevant medical form must be completed. No medication can be issued in school without the appropriate form. Please see below a flow chart to help you determine which one to use and a link to the forms. Please contact the school if you are still unsure which medical form to use. Copies of the forms are also available at the school office. 

MEDICAL FORMS

There are a range of forms required to ensure that we can provide medication to learners.  Please view the forms below.  We can support you in school to print and complete any medical form.

Form 1 (if you want school to administer short-term non-prescribed medication)
Form 2(if you want school to administer short-term prescribed medication)
Form 3(if you want school to issue long-term prescribed medication)
Form 4 (if you want school to issue long-term ‘as required ‘prescribed medication)
Form 5 (If you want your child to self-administer e.g. inhalers)

Any forms completed at home should be returned to the school office.
